3K a year 2 days ago 2 days ago 2 days ago The Rapid Rehousing Case Manager will provide financial assistance and case management support to the literally homeless meeting qualifications for the Rapid Rehousing Program.
Client participation in program is referral-based only through the county's coordinated entry system; eligibility determined by assessments and need.
Case Manager will work with qualified individuals and families to attain self-sufficiency through intensive case management, community referral linkages, support in building life skills and setting short- and long-term goals that work to address barriers surrounding homelessness.
Responsibilities & Duties Differentiate if client is in need of short- or long-term assistance Conduct full SPDAT on clients who have been referred to program Assist client with housing search Address barriers hindering Housing-First Model (vital documents, criminal record, etc.
) Communicate with landlords and eligible clients about program qualifications and requirements Perform initial and annual home inspections Assist in processing lease agreements with landlords, families, and contractors Determine client copayment amounts Monthly case meetings with clients actively enrolled in program with periodic home visits Maintain client data in the Homeless Management Information System (HMIS) and report regularly on client progress and goal attainment Work with clients to set goals and implement strategies to meet goals Network with community service providers to identify programs and resources for clients Attend community meetings, HMIS trainings, and all staff meetings Participate in on-going training and professional development opportunities Other duties as assigned Qualifications Associates Degree in psychology, sociology, or related field (preferred) or 2-5 years of relevant experience working in field or with homeless and/or low-income individuals and families.
Willing to train the right individual.
Excellent written and verbal communication skills Ability to multi-task and juggle competing priorities effectively Strong organizational skills, attention to detail, and ability to meet deadlines consistently Ability to work effectively both independently and as part of a team Ability to build strong relationships with a diverse range of clients, colleagues, community members and agencies Commitment to an empowering, client-centered approach to case management Compassion for people who are struggling with a variety of challenges, including homelessness, mental illness and addiction Job Type:
